      Dr. Nasseh can you please give me an advice on this two visit case. 65 yr female with buccal swelling around #13 pt asymptomatic,was not even aware of swelling. (-)to Endo ice, (+) percussion and PAP lesion apically. Access the canal and flush out visual exudate. Cleaned and medicated and Rx antibiotics. After two weeks she is swollen again but reports no pain. Not sure what do next. If to drain and med again or Ext. I didn’t see any internal fractures. Please advise. Thank you. Dr. B Lemus

      @Dr. Root 33: it would be helpful to take aCBCT to make sure there not another buccal or lingual canal here that was not missed during access. If no additional canal, making sure you have adequate Apical engagement is helpful. A tooth like this is at least a size 45 of not larger due to Apical resorption it is exhibiting.

      Thank you for your recommendation. So a CT scan was taken and found 14 to have pathology as well, that didn’t show well on the initial radiograph.

      @DrRoot33: Makes sense since it’s had a crown without Endo and maxillary teeth don’t show as well radiographically. That’s why CBCT is extremely helpful in such cases. Best Wishes.
